Before your first visit
Please take a few moments to fill out the intake form, which you can print from below. Bring your responses, along with any recent blood work or diagnostic test results you may have, to your initial consultation so that we can discuss your health history and concerns in detail. If you do not have access to a printer, please let us know, and arrive 20 minutes before your appointment to fill out the form in our waiting room.
